Summary
A community outreach coordinator resume is a one to two page document showing how many people you reached, how many enrolled, how many stayed, and whether the grant deliverables were met. This guide gives you three adaptable versions, the reach funnel most applicants leave off, and current Bureau of Labor Statistics pay and outlook data.
Community Outreach Coordinator resume examples by experience level
In short, a community outreach coordinator resume is a one to two page document that reports measured reach: how many people you contacted, how many of them you actually reached, how many enrolled, how many stayed, and whether the funder's deliverables were met.
Almost every resume in this occupation describes effort instead. Instead, most files list events held, flyers distributed, partners engaged, awareness raised. None of those survive contact with a program officer, because none of them can be checked against a grant report.
Resume guide for a community outreach coordinator resume
For that reason, this guide and the corresponding community outreach coordinator resume example will cover:
- First, how to write a community outreach coordinator resume, section by section
- Then, the five-stage reach funnel and the conversions between each stage
- Also, three adaptable versions: outreach worker, coordinator, and grant portfolio manager
- In addition, how to write partnerships, deliverables and the funder's reporting cycle
- Finally, what the job market looks like and what you can expect to earn
How to write a community outreach coordinator resume
Six sections: contact header, summary, the reach funnel, outreach and program experience, education and credentials, and skills including languages.
The funnel sits directly under the summary because it is the only part of the page that can be verified against a document a funder already holds. In other words, everything below it explains how the funnel was produced.
| Section | What it is answering | Where it goes |
|---|---|---|
| Reach funnel | How many people did this person actually move through a program? | Directly under the summary |
| Partnerships | Can they open doors that stay open? | Inside the funnel block, with a renewal figure |
| Outreach experience | What methods produced those conversions? | Reverse chronological, with rates |
| Credentials and languages | Do they clear the funder's staffing requirement and speak the community's language? | Together, near the education block |
A percentage without its denominator is decoration
"Increased program participation by 40 percent" is the most common sentence in outreach resumes and one of the least informative. Forty percent of what, measured how, over what period.
Write the stack instead: "1,840 contact attempts produced 1,110 reached, 402 screened eligible, 268 enrolled and 191 completing the full sixteen sessions, across the 2025 program year." A reader can compute every conversion themselves, which is why they believe it.
If your program is small, the small numbers are still better than a percentage. A funnel of 96 contacts and 31 enrollments is a real, checkable claim. A 40 percent increase on an unnamed base is not.
Run the finished file through the ATS resume checker before you submit it. Health departments, hospital systems and grant-funded nonprofits all parse applications into a structured record before anyone reads them.
Choosing the best resume format for a community outreach coordinator resume
Reverse chronological, one page until you manage staff or a grant portfolio. Outreach roles are usually funded for a fixed term, so short stays are normal and expected here in a way they are not in other occupations.
Also, where a term contract ended, say so in the role line: "Outreach Coordinator, 2021 to 2024, three-year state prevention grant, program sunset at award end." Left unexplained, four two-year roles read as a pattern. Explained, however, they read as four completed grants.

Similarly, put the languages in the header, not at the bottom of a skills list. In this occupation the language you work in is a hiring requirement more often than a nice-to-have, and a reader scanning for it should not have to reach page two.
Make use of a summary
In practice, four lines: the population and geography, the funnel in numbers, the deliverable against its target, and the credential.
Community health worker running door-to-door and clinic-based outreach for a diabetes prevention program across four south Tucson zip codes. Produced 1,110 reached contacts from 1,840 attempts in the 2025 program year, of which 402 screened eligible, 268 enrolled and 191 completed the full sixteen-session curriculum, a 71 percent completion rate against a program target of 60. All four grants met or exceeded their annual enrollment deliverables in the last reporting year, with the portfolio reporting 4,900 enrollments against a combined target of 4,150. Lead eleven outreach staff, own the monthly data submission and the annual continuation applications, and hold an MPH and the CHES credential.
Right vs wrong: the same community outreach coordinator summary, twice
| ✅ Right | ❌ Wrong |
|---|---|
| Delivered 1,480 enrollments against an annual grant deliverable of 1,200. | Exceeded program goals and expectations consistently. |
| 9,400 contact attempts produced 5,600 reached contacts and 1,090 retained at six months. | Conducted extensive community outreach and engagement activities. |
| 19 active partner agreements, 17 of them renewed at term. | Built strong relationships with community partners and stakeholders. |
In contrast, the right column is what a program officer already has in their file. The left column is what most applicants send instead.
Outline your outreach experience
Organization, program, funder, geography and dates. Then three to five bullets, each carrying a stage of the funnel, a conversion, or a deliverable against its target.
Above all, never name or describe an identifiable participant. Describe the community and the volume.

Outreach Manager, Community Health Programs, Pima Health Access Partnership, Tucson, AZ, March 2024 to Present
Own a four-grant portfolio worth $2.4 million a year across chronic disease prevention, maternal health, immunization access and behavioral health navigation, with eleven outreach staff across Pima and Santa Cruz counties.
Portfolio delivered 4,900 enrollments against a combined annual target of 4,150; all four grants met or exceeded their enrollment and retention deliverables in the last reporting year.
Raised the reached-to-enrolled conversion across the portfolio from 22 percent to 29 percent by standardizing the eligibility screen and moving first contact from flyer to warm referral.
Hold 31 active partner agreements, 27 of them renewed at term, including three clinic referral pathways that now supply 38 percent of all enrollments.
Own the monthly data submissions, the quarterly progress narratives and the four annual continuation applications; no finding on the last two funder site visits.
Report the funnel, not the effort
After all, outreach is the one job in social services where the whole of your work can be counted, and counting it is the thing that separates a hireable resume from a busy one.
The five stages. Report every stage you have a number for, in order, with the conversion between them. If you only have three of the five, give three and label them honestly.
| Stage | What it means | What to write |
|---|---|---|
| Attempted | Contacts you initiated: doors, calls, texts, tables, clinic referrals | The raw number and the method mix |
| Reached | People you actually spoke with | The number and the attempted-to-reached rate |
| Screened or eligible | People assessed against program criteria | The number and what the criteria were |
| Enrolled | People who started the program | The number and the reached-to-enrolled rate. This is the figure funders count. |
| Retained or completed | People still there at the program's measure | The number, the measure point, and the rate |
In particular, the middle conversion is the one experienced readers go to first. That is because attempted-to-reached tells them about your method and your hours. Reached-to-enrolled tells them whether you can actually move someone from a conversation into a program, and that is the skill being hired.
Outreach deliverables, partnerships and reporting
Deliverables against target. Grant-funded outreach runs on a number written into an award document. Put yours on the page with the target beside it: "1,480 enrollments against an annual deliverable of 1,200." Under target is not automatically a failure, but it has to be explained in one clause: "980 against a 1,200 deliverable, after the award's second site was delayed two quarters."
Partnerships, with renewals. A signed agreement is easy. A renewed one means the partner saw value. Give both counts and, if you have it, the share of enrollments that came through partner referral, because that is the number that proves a partnership was operational rather than ceremonial.
The reporting cycle. Say which one you ran. Monthly data submissions, quarterly progress narratives, annual continuation applications, federal financial reports, site visits. A coordinator who has carried a reporting calendar without a finding is a much lower-risk hire than one who has only done the outreach and handed the numbers to someone else.
Reach and results, 2025 to 2026 program year
Funnel: 9,400 contact attempts, 5,600 reached (60 percent), 2,310 screened eligible, 1,480 enrolled (26 percent of reached), 1,090 retained at six months (74 percent).
Method mix: 41 percent clinic warm referral, 28 percent door-to-door canvass, 19 percent community event tabling, 12 percent text and phone follow-up.
Deliverable: 1,480 enrollments against an annual grant target of 1,200, or 123 percent of target.
Partnerships: 19 active agreements with clinics, schools, faith organizations and a tribal health program. 17 renewed at term. Partner referral supplied 41 percent of enrollments.
Reporting: 12 monthly data submissions, 4 quarterly progress narratives, 1 annual continuation application, all filed on time. No finding at the 2026 funder site visit.
Languages and communities: Spanish and English, working across four south Tucson zip codes and two rural Santa Cruz County communities.
Community outreach credentials to list
The credential. Two sit over this work and they are not interchangeable. The Certified Health Education Specialist credential from the National Commission for Health Education Credentialing requires a bachelor's, master's or doctoral degree plus either a transcript showing a major in health education or at least 25 semester credits of coursework across the Eight Areas of Responsibility (NCHEC, CHES exam eligibility, 2026). Community health worker certification is a state matter: twelve states operate a state-administered program, Arizona among them, with a further eleven running certification through private boards or CHW associations and at least eight more developing one (Association of State and Territorial Health Officials, March 2025). Name yours, name the issuing state or body, and give the number.
Build a snapshot of your key skills
Ten to fourteen entries in three groups: outreach method, data and reporting, and the community-facing skills including language.
Outreach method: Door-to-door canvassing, Clinic warm referral pathways, Community event tabling, Text and phone follow-up campaigns, Faith and school partnership outreach, Promotora model delivery
Data and reporting: Funnel tracking and conversion analysis, REDCap data entry and export, Grant deliverable tracking, Quarterly progress narratives, Continuation applications, Site visit preparation
Program and community: Health literacy and plain-language materials, Bilingual facilitation in Spanish and English, Cultural mediation, Group session facilitation, Motivational interviewing, Participant retention design
Tools: REDCap, Salesforce Nonprofit Cloud, Microsoft Excel and Power BI, Canva for bilingual materials
List your education and certifications
Degree, then the credential with its issuer and number, then the trainings a funder recognizes.
Master of Public Health, Health Behavior and Health Promotion, University of Arizona, Tucson, AZ, 2024.
Bachelor of Science, Public Health, University of Arizona, Tucson, AZ, 2018.
Certified Health Education Specialist (CHES), National Commission for Health Education Credentialing, #CHES-22418, issued 2022.
Community Health Worker certification, State of Arizona, issued 2019, current.
Training: National Diabetes Prevention Program lifestyle coach training, CDC-recognized (2020). Motivational Interviewing, three-day MINT-endorsed training (2022). REDCap data management (2023). Plain language and health literacy, Institute for Healthcare Advancement (2021).
By comparison, the typical entry-level education for health education specialists is a bachelor's degree (BLS Occupational Outlook Handbook, Health Education Specialists and Community Health Workers, updated 27 August 2026). Community health worker postings frequently require no degree at all and select instead on language, community ties and certification, which is why the header of your resume matters more than the education block.
Choose the right layout and design
In short, single column, 11 or 12 point body type, plain headings, no photograph. Above all, the funnel block should be the most scannable thing on the page: one line per stage, numbers aligned, conversions in parentheses.
Give every stage of the funnel you have a number for, in order. Put the deliverable next to its target. Give partnership counts with renewals. Name the reporting cycle you personally ran. Put your languages in the header. Label term contracts with the grant and the sunset date.
Do not write a percentage without its base number. Do not count events held or flyers distributed as results. Do not write "engaged the community" anywhere. Do not describe an identifiable participant. Do not present enrollments as retention; they are different stages and a funder knows it.
Community outreach coordinator job market and outlook
In fact, outreach coordinator postings sit across two federal occupations. Health education specialists is the closer match for coordinator and manager titles; meanwhile, community health worker is the closer match for frontline outreach roles.
| Measure | Value |
|---|---|
| Health education specialist jobs, 2025 | 70,400 |
| Projected change, 2025-35 | 6 percent, +3,900 |
| Projected annual openings | About 6,900 |
| Typical entry-level education | Bachelor's degree |
| Median annual wage, health education specialists, May 2025 | $64,070, or $30.80 an hour |
| Median annual wage, community health workers, May 2025 | $51,850 |

The setting is worth more than the title
Median annual wages for health education specialists varied by more than $32,000 across settings in May 2025: hospitals paid a median of $83,040, government excluding state and local education and hospitals $69,740, educational services $62,370, ambulatory healthcare services $60,500, and social assistance $50,230 (BLS Occupational Outlook Handbook, May 2025).
The occupation's overall median was $64,070, with the lowest 10 percent under $42,990 and the highest 10 percent above $118,200 (BLS, May 2025). Community health workers, doing frontline outreach in many of the same programs, had a median of $51,850.
A third federal line matters here too, because outreach postings in community agencies are often classified under social and human service assistants, with a median of $45,930 in May 2025 (BLS Occupational Outlook Handbook, Social and Human Service Assistants, May 2025). The same work carries three different pay lines depending on who writes the job description, which is the strongest practical argument for putting a funnel and a credential on your resume rather than a job title.
Where the jobs sit:
| Setting | Share of employment | Median annual wage (May 2025) |
|---|---|---|
| Government, excluding state and local education and hospitals | 24 percent | $69,740 |
| Ambulatory healthcare services | 18 percent | $60,500 |
| Hospitals, state, local and private | 16 percent | $83,040 |
| Educational services, state, local and private | 14 percent | $62,370 |
| Social assistance | 7 percent | $50,230 |

What salary you can expect as a community outreach coordinator
To begin with, health education specialists had a median annual wage of $64,070, or $30.80 an hour, as of May 2025, with the lowest 10 percent under $42,990 and the highest 10 percent above $118,200 (BLS Occupational Outlook Handbook, May 2025). Meanwhile, community health workers had a median of $51,850 in the same cycle.
The band you land in is decided less by your years than by three facts you can put on a page. The first is the setting: a hospital line and a social assistance line are more than $30,000 apart for similar work. The second is the credential, because grant staffing plans and health system job families both reference them by name. The third is whether you own the reporting, since a coordinator who files the continuation application is budgeted differently from one who supplies numbers to whoever does.
Term-funded roles complicate the picture in one useful way: a coordinator who has carried a grant through renewal is materially easier to hire than one who has not, because renewal is the risk the employer is actually managing. If you have been through one, say so, with the year and the award.
Key takeaways for a community outreach coordinator resume
- First, report the funnel in stages, with the conversion between each one.
- Then never give a percentage without the number it was calculated from.
- Next, put the grant deliverable next to its target, and explain a shortfall in one clause.
- Similarly, give partnership counts with renewals, and the share of enrollments they supplied.
- Also, name the reporting cycle you personally ran, and any clean site visit.
- Finally, put your languages and the communities you serve in the header.
- Name your credential, its issuer and its number, because the state rules differ.

Community outreach coordinator resume questions, answered
How long should a community outreach coordinator resume be?
One page until you manage staff or a grant portfolio, two after that. The funnel block earns its space at either length. Cut a second example of the same outreach method before you cut a stage of the funnel.
What if my program never tracked a full funnel?
Report the stages you can evidence and label the gap. "Enrollment and six-month retention tracked; contact attempts not recorded" is an honest, professional line, and it tells a reader something useful about the program you came from. Inventing an attempted figure is the one mistake that a program officer will spot instantly, because the conversions will not behave like real ones.
Do I need the CHES credential?
Not for most frontline roles, and increasingly yes for coordinator and manager posts in health systems and health departments. Eligibility requires a bachelor's, master's or doctoral degree plus either a health education major or at least 25 semester credits across the Eight Areas of Responsibility (NCHEC, 2026). If you are gathering credits toward it, put the count on the page.
Is community health worker certification the same thing?
No. It is a state credential, not a national one, and it is not offered everywhere. Twelve states run a state-administered program, eleven more certify through private boards or CHW associations, and at least eight are developing one (Association of State and Territorial Health Officials, March 2025). Always write the issuing state next to it, because a reader in another state cannot assume it transfers.
How do I write outreach experience from volunteer or campaign work?
Exactly the same way. A voter registration drive, a mutual aid network or a church health ministry all produce a funnel: attempts, reached, signed up, showed up. Write the numbers, the geography, the method mix and the period. Unpaid outreach with a real funnel beats paid outreach described as engagement.
Should I list the languages I speak even if the posting does not ask?
Yes, in the header, with an honest level. In this occupation language is often the reason a program can reach a community at all, and it is one of the few resume facts that gets an applicant moved into a different pile rather than further down the same one.
