A physician assistant cover letter is a one-page letter that tells a medical director which service you can join, what you do without a physician at the bedside, and how long it will take to credential you.
Those three facts are what the reader is actually deciding. Everything else in the letter is context for them.
Guide to a physician assistant cover letter
This guide and the corresponding physician assistant cover letter example will cover:
- How to structure the letter, paragraph by paragraph
- Why the specialty and the volume belong in the first two sentences
- What counts as evidence when you cannot describe a patient
- How the letter changes when you are moving between specialties
- The openings that get a strong file read rather than filed

Put the credentialing answer in the first paragraph
A medical director reading PA applications is running a mental clock. How long until this person is on the schedule unsupervised? Everything in the file either shortens that estimate or leaves it where it is.
Three sentences shorten it: the specialty with a duration, the volume you carry, and the state license with its status. "PA-C since 2018, five years in emergency medicine at a Level II trauma center, 2.1 patients per hour, Idaho license active with no restrictions" answers more than a page of adjectives.
Then name the procedures you do unassisted. Privileging is granted procedure by procedure, and a letter that already lists them has done part of the reader's paperwork.
A physician assistant cover letter example you can adapt
Adaptable physician assistant cover letter example
Dear Ms. Ellery,
I am applying for the Orthopaedic Surgery Physician Assistant post at Highline Surgical Partners. I have been PA-C certified since 2018 and hold an active Idaho license with no restrictions and full schedule II to V prescriptive authority with current DEA registration. My last five years have been in emergency medicine at a Level II trauma center, carrying 2.1 patients per hour, with fracture reduction, splinting and joint injection among the procedures I perform without a physician at the bedside.
The part of that work most relevant to your service is the musculoskeletal volume. In the last twelve months I performed 240 laceration repairs, 95 fracture reductions and 140 ultrasound-guided peripheral lines, and I ran the department's splinting competency for new staff. I also restructured the first-hour rooming order in our fast-track area with the charge nurse, which took door-to-provider time from 41 minutes to 24 over one quarter. I am used to being measured on throughput and on whether the next clinician has to redo my work.
I noticed Highline runs a same-day fracture clinic alongside the operative schedule, which is the pattern I would most like to work in, and that you added a second arthroplasty room this year. I have started the NCCPA Certificate of Added Qualifications in Orthopaedic Surgery and have 48 of the required Category I CME credits logged. I would be glad to take the postoperative clinic day and the on-call rotation from the start.
My credentialing file is current and I can submit it the day an offer is made, including the collaborative practice agreement template my current department uses. I can start with four weeks' notice. Thank you for your time.
Sincerely,
Tobias Fenwick, PA-C

What counts as evidence when you cannot describe a patient
Nothing in a PA letter should be recognizable as one person. Describe the work and the group instead.
Kind of evidenceWhat it looks like in a sentence
- VolumeCarry 2.1 patients per hour, roughly 3,900 encounters a year
- Procedure counts240 laceration repairs, 95 fracture reductions and 140 ultrasound-guided lines in twelve months
- Panel workLifted the share of panel patients with an A1c in the last 12 months from 62% to 81%
- ThroughputFast-track door-to-provider time from 41 minutes to 24 in one quarter
- ScopeIndependent disposition authority for ESI 3 to 5 under a written practice agreement
- TeachingPrecepted 14 PA students; wrote the procedural sedation checklist used on every shift
When you are changing specialty
Do
- Lead with the procedures and clinical problems that carry across, with counts.
- Say what you have already done to prepare: CME credits logged, shifts shadowed, a Certificate of Added Qualifications started.
- Give the reader a realistic runway rather than claiming you need none.
- Name the state license and prescriptive schedules, since those do not change with specialty.
- Read their service pages and name one real thing about how they run.
Don’t
- Do not apologize for the move or call it a career change; more than half of practicing PAs have made one.
- Do not claim competence in a procedure you have performed a handful of times, because privileging asks for numbers and the discrepancy surfaces later.
- Do not write "under the supervision of" if your state has moved to collaborative or independent language.
- Do not send a letter with no service name in it.
Why the specialty sentence is worth money
The Bureau of Labor Statistics reports a median annual wage for physician assistants of $135,880 as of May 2025, with the lowest 10 percent under $99,380 and the highest 10 percent above $190,280 (BLS, May 2025).
The settings sit far apart inside that range. Offices of physicians employed 53 percent of PAs at a median of $132,000, while outpatient care centers employed 9 percent at $153,500 and government 3 percent at $157,040 (BLS, Occupational Outlook Handbook, May 2025).
Employment is projected to grow 21 percent from 2025 to 2035, much faster than average, with about 11,500 openings a year over the decade (BLS, 2025-35 projections). In a market moving that fast, the letter that names a specialty and a volume is not competing for the same job as the letter that says "physician assistant, seven years".
Length, format and sending it
One page, four paragraphs, 275 to 400 words. PDF unless the portal says otherwise, named for yourself and the post: tobias-fenwick-orthopaedic-pa-cover-letter.pdf.
Address a person. Service leadership and lead advanced practice providers are usually named on a hospital's own pages, and "Dear Hiring Manager" on an otherwise specific letter reads as a file sent to twelve systems.
Keep every figure identical to the one on your resume. Credentialing verifies what you write, and two documents that disagree slow a file more than a weak sentence does. Our physician assistant resume example uses the same numbers as this letter.
Key takeaways
- Name the specialty, the duration and one volume figure in the first two sentences.
- State the license, its status and the prescriptive schedules; they are the gate.
- List the procedures you perform unassisted, because privileging is granted procedure by procedure.
- Give one measured result with a time frame attached.
- If you are switching specialty, show the preparation you have already logged.
- Name one real thing about their service, taken from their own pages.
- Say when your credentialing file can be submitted and when you can start.
The physician assistant resume example uses the same story, the same numbers and the same header.
Physician assistant cover letter questions, answered
How long should a physician assistant cover letter be?
One page, four paragraphs, 275 to 400 words. Medical directors read these between clinical sessions, and length does not read as effort.
Do I need a cover letter if the health system uses a portal?
Attach one wherever there is a field for it. The portal collects the same structured data from everybody. The letter is the only place you can say which service you can join, what you do unassisted, and how quickly your credentialing file can move.
What do I write as a new graduate?
Write the clinical year as a job. Name the nine rotations, the total supervised clinical hours, and the procedure counts from the year. Then the certification, the license and its status, and whether the practice agreement is in place. A new graduate who has already filed their agreement and DEA registration is a materially easier hire than one who has not started.
Should I mention my supervising or collaborating physician?
Describe the agreement rather than the person. Say what it authorized you to do independently and what triggered a consultation. That tells a reader how much oversight your next post has to build around you, which is the question behind the question.
How do I handle a gap in clinical practice?
State it plainly with dates, then say what you did to stay current: CME credits logged, certification maintenance kept up, per diem or locum shifts, a refresher course. A PA whose NCCPA certification stayed current through a gap has a much shorter path back onto a schedule, so make that explicit rather than leaving it to be inferred.
